Introduction

Urinary tract infection (UTI) is one of the most common types of infection. Currently, diagnosis is primarily based on microbiologic culture, which is time- and labor-consuming. The aim of this study was to assess the diagnostic accuracy of urinalysis results from UriSed (77 Electronica, Budapest, Hungary), an automated microscopic image-based sediment analyzer, in predicting positive urine cultures.

Materials and methods:

We examined a total of 384 urine specimens from hospitalized patients and outpatients attending our hospital on the same day for urinalysis, dipstick tests and semi-quantitative urine culture. The urinalysis results were compared with those of conventional semi-quantitative urine culture.

Results:

Of 384 urinary specimens, 68 were positive for bacteriuria by culture, and were thus considered true positives. Comparison of these results with those obtained from the UriSed analyzer indicated that the analyzer had a specificity of 91.1%, a sensitivity of 47.0%, a positive predictive value (PPV) of 53.3% (95% confidence interval (CI) = 40.8–65.3), and a negative predictive value (NPV) of 88.8% (95% Cl = 85.0–91.8%). The accuracy was 83.3% when the urine leukocyte parameter was used, 76.8% when bacteriuria analysis of urinary sediment was used, and 85.1% when the bacteriuria and leukocyturia parameters were combined. The presence of nitrite was the best indicator of culture positivity (99.3% specificity) but had a negative likelihood ratio of 0.7, indicating that it was not a reliable clinical test.

Conclusions:

Although the specificity of the UriSed analyzer was within acceptable limits, the sensitivity value was low. Thus, UriSed urinalysis results do not accurately predict the outcome of culture.  相似文献

This article examines the perceptions of education in Turkey, which refer to a nebulous package of formal education and a cultured stance. Guided by the literature on symbolic violence, we argue that underprivileged groups misrecognize arbitrary hierarchies by considering them just and inevitable. Elite tastes have been internalized by other groups in a particular historical context of education and culture. We investigate the historical roots of this seemingly ahistorical constellation of power relations around education and then consider the implications for the neoliberal period. Then, we contextualize the responses to symbolic violence. Subordinate groups complicate the effects of symbolic violence by exhibiting diverse responses that range from outright submission to implicitly questioning cultural and moral boundaries, creating class and ethnic others in the process. This occurs by constructing cultural and moral boundaries, especially targeting the ‘vulgar’ culture of celebrities and Kurds.  相似文献

Casual games disrupted the games industry, but not in ways commonly believed. What if we left behind the hardcore vs. casual games dichotomy to reveal that casual gameplay and casual game development have extended the neoliberal and neocolonial logic of the industry? Casual games, in terms of design and industry practices, remind us that there is nothing inherently liberating about play. Rather, the design and development practices of casual games should be understood as an extension and acceleration of neoliberal and neocolonial logics. Casual gameplay and casual game development pull us within processes of cruel optimism. These deeply political economic processes endanger free play and creativity and therefore are obstacles to the flourishing of gamers and game developers as free subjects. In this neoliberal and neocolonial game market, cruel optimism is enticing because casual gameplay and game development emerge as powerful actors and practices in a context where the state has globally failed in the distribution of hope.  相似文献

This study investigated the performance of four widely used data-collection designs in detecting test-mode effects (i.e., computer-based versus paper-based testing). The experimental conditions included four data-collection designs, two test-administration modes, and the availability of an anchor assessment. The test-level and item-level results were analyzed with inferential statistics and multidimensional scaling, respectively. The test-level results supported the superiority of the single-group counterbalanced design and the random-groups design over the single-group design without counterbalancing and the anchor test design in the recovery of the actual test-mode effects. Analysis at the item level revealed the presence of a two-dimensional solution, with the data-collection design contributing significant variability over and beyond the test mode.  相似文献

In low-stakes assessments, some students may not reach the end of the test and leave some items unanswered due to various reasons (e.g., lack of test-taking motivation, poor time management, and test speededness). Not-reached items are often treated as incorrect or not-administered in the scoring process. However, when the proportion of not-reached items is high, these traditional approaches may yield biased scores and thereby threatening the validity of test results. In this study, we propose a polytomous scoring approach for handling not-reached items and compare its performance with those of the traditional scoring approaches. Real data from a low-stakes math assessment administered to second and third graders were used. The assessment consisted of 40 short-answer items focusing on addition and subtraction. The students were instructed to answer as many items as possible within 5 minutes. Using the traditional scoring approaches, students’ responses for not-reached items were treated as either not-administered or incorrect in the scoring process. With the proposed scoring approach, students’ nonmissing responses were scored polytomously based on how accurately and rapidly they responded to the items to reduce the impact of not-reached items on ability estimation. The traditional and polytomous scoring approaches were compared based on several evaluation criteria, such as model fit indices, test information function, and bias. The results indicated that the polytomous scoring approaches outperformed the traditional approaches. The complete case simulation corroborated our empirical findings that the scoring approach in which nonmissing items were scored polytomously and not-reached items were considered not-administered performed the best. Implications of the polytomous scoring approach for low-stakes assessments were discussed.  相似文献
